Death investigations & reports

This page contains details of death investigations that are currently in progress as well as final reports. The PPO is only able to publish final reports after sharing them with the family of the deceased person and after the coroner’s inquest has taken place. Please visit our archive site to view reports published before June 2024. You can also view investigations started before July 2024 too.
