Naming children in our fatal incident reports

As of today, the Prisons and Probation Ombudsman will now name children in our fatal incident reports and on our website, aligning our approach with our existing practice of naming adults whose deaths we investigate. 

Ombudsman Adrian Usher said: “This information is already in the public domain by the time cases reach the Coroner. I have carefully consulted with stakeholders and bereaved families on this matter, and I believe this is a more transparent approach.” 

The PPO will continue to not name the deaths of babies born to mothers in prison. The deaths we are investigating and our final investigation reports can be found on our website here
