The prisoner complained that some of their property had been lost when they transferred from HMP Wymott to HMP Risley. Neither prison was able to provide any property cards, so it was not possible to confirm whether the Prisoners’ Property Policy Framework had been complied with. In addition, the prisoner’s complaint had been responded to a month late by Wymott.
Recommendations/outcome:
IPCI recommended payment of compensation by Risley, and also recommended that Wymott apologise to the prisoner for the way their complaint was dealt with and to ensure they comply with the mandatory requirements of the Prisoner Complaints’ Framework.

The prisoner complained that their property had been lost by HMP Standford Hill after they transferred to HMP The Mount. They complained that were not given the chance to pack up their property and that items that were recorded on their property cards at Standford Hill, did not arrive at The Mount. Our investigation found that Standford Hill had used a local version of a Cell Clearance Certificate, contrary to the Prisoner’s Property Policy Framework, and there were discrepancies between the Cell Clearance Certificate and the prisoner’s property card.
Recommendations/outcome:
HMPPS agreed to compensate the prisoner for his missing property following the submission of IPCI’s report. They also agreed to ensure that staff at Standford Hill comply with the requirements of the Prisoners’ Property Policy Framework including the use of the mandatory Cell Clearance Certificate (F2056J) rather than the local version being used.

The prisoner complained about several incidents of bullying and victimisation by both staff and prisoners. Our investigation found the prison had not conducted adequate investigations into the allegations nor kept adequate records and therefore, had no evidence of reasonable measures taken to reassure of the prisoner’s safety.
Recommendations/outcome:
We recommended that prison investigate the allegations, maintain appropriate records of the investigation, and consider any actions necessary to reassure the prisoner of his safety. We also asked that the prison provided the outcome of the investigation to IPCI.

The prisoner complained that during a routine security search, his trousers had been pulled down in-front of a large group, causing him to feel humiliated and embarrassed. The prisoner said that no apology was issued.
Recommendations/outcome:
Following IPCI involvement, the prisoner was issued an apology from the CM which he had accepted. We outlined the benefits of professional working relationships to HMP/YOI Stoke Heath.

The prisoner complained about a body search conducted in October 2023. He was concerned that his intimate body parts were captured on the body worn video camera (BWVC) footage.
Recommendations/outcome:
We recommended that the prisoner was issued with a letter of apology. We asked that the Governor commission an urgent review of the prison local BWVC policy to ensure it is in line with national policy and to ensure that full searches are not routinely filmed on BWVCs.

The prisoner complained about the actions of a named member of staff, who he felt was bullying him. The IPCI investigation established that the prison had taken the matter seriously and investigated it. In our investigation we found evidence of some staff behaviour which was unprofessional, but this did not meet the threshold for an investigation under the relevant policy.
Recommendations/outcome:
We wrote to the Governor to alert him to the behaviour and to ask him to consider whether there were any wider concerns regarding this member of staff.

The prisoner complained that his progression was delayed due to not having an up-to-date sentence plan and OASys. The IPCI investigation established that there were significant delays in these documents being completed, which would have resulted in a delay to him being recategorised down, if eligible.
Recommendations/outcome:
Since the complaint was accepted for investigation, the documents had subsequently been reviewed. The decision remained that he was in ineligible to be recategorised down.

The prisoner complained that having been granted category D conditions, the decision was quickly reversed when further information emerged. The IPCI investigation established that the decision was made before some further important information was identified
Recommendations/outcome:
We recommended the prison apologise to the complainant and issue a notice to staff that recategorisation decisions should not be made and communicated until all of the relevant information has been obtained and considered.
Upheld complaint against the Governor of HMP Leeds

The prisoner complained that ‘follow on’ property from HMP Leeds failed to arrive. The IPCI investigation established that prior to transfer the prisoner packed his property, but that the contents had not been checked against the property cards to ensure what the bags contained. This was in breach of policy.
Recommendations/outcome:
We were critical of the inadequate responses to the prisoner’s complaint and asked the Governor to remind staff of the requirements of the property framework regarding checking property on transfer. We asked that the prisoner be paid £120 in compensation.

The prisoner complained about damage to his stereo when he transferred between Ranby and Bullingdon. He initially complained to Geo Amey who investigated and did not uphold his complaint. Neither Ranby or Bullingdon investigated the matter, neither demonstrated good complaint handling practice, or adherence to the requirements of the property framework to audit the property.
Recommendations/outcome:
We recommended each prison contributed equally to the cost of a replacement stereo.
